Background
Bangladesh's population is aging rapidly due to increased life expectancy, with projections indicating that by 2040, one in five Bangladeshis will be 60 years or older. This growth is driven by a declining fertility rate and rising life expectancy. Key health challenges include a high burden of non-communicable diseases (NCDs), accounting for 70% of deaths, and inadequate healthcare system capacity to address the specific needs of older adults, such as long-term care and chronic disease management.
Key Findings
The study identified significant gaps in Bangladesh's healthcare system for supporting an aging population using the FIRE framework:
- Financing: NCD control budgets are underfunded (only USD 0.82 per capita vs. USD 1.50 recommended by WHO), leading to financial strain on households. Budget allocations are inefficient.
- Innovation: Primary healthcare (PHC) is acute-focused and lacks protocols for older adults; digital health and telemedicine are underutilized, and data collection is fragmented.
- Regulation: Policies for older adults exist but are poorly enforced; workforce and supply chain issues are prevalent, with inadequate training for health workers and frequent drug shortages.
- Evaluation: There is a shortage of human resources for monitoring, and evaluation systems are weak, hindering accountability for care.
Recommendations
To address these challenges, the study proposes the following FIRE-based reforms:
- Financing: Allocate dedicated budget for senior care via output-based financing and cross-ministerial collaboration to reduce costs.
- Innovation: Implement digital health tools for better data management and telemedicine to broaden access. Enhance PHC with age-sensitive services, including geriatric syndrome screening and assistive devices.
- Regulation: Strengthen financial and clinical oversight through training programs and supply chain management. Incorporate geriatric palliative care into national training and regulations.
- Evaluation: Establish specialized units for monitoring and periodic health workforce evaluations to ensure program adherence.
Conclusion
Bangladesh must strategically reform its healthcare system to address the needs of an aging population, prioritizing resource allocation, age-friendly infrastructure, and integrated care at PHC levels. These reforms are urgent to achieve Sustainable Development Goals and Universal Health Coverage, ensuring financial, access to care, and service quality for older adults.
